Audit trails that support accountable decisions

UNDERSTAND THE CONCEPT

An audit trail records relevant actions and changes with identity, time and context. Useful events include approvals, revisions, permission changes and exports. A timestamp alone does not explain the business reason for a change.

Define retention, access and protection against unauthorized alteration. Log enough information to reconstruct a decision without unnecessarily collecting sensitive content. Make correction history visible and distinguish business audit records from technical troubleshooting logs.

Questions to take back to your team

  1. Which decisions and changes require an audit event?
  2. Can the sequence be reconstructed reliably?
  3. Who can access and administer the audit records?
